Assessment Participation Consent

This Assessment Participation Consent ("Consent") describes the ILCI-21 Leadership and Culture Index assessment, explains how your responses will be used, and outlines your rights as a participant. Please read this document carefully before completing the assessment. By accepting these terms within the platform, you confirm your voluntary and informed consent to participate.

ILCI-21: Isozaki Leadership and Culture Index is a self-report leadership and culture assessment developed by Isozaki Coaching LLC. It consists of 21 questions about individual and collective perceptions of leadership, culture, and the systems that support both across three domains and nine subdomains:

Overall scores are interpreted through five developmental bands: Flourishing, Growing, Emerging, Withering, and Uprooted. These bands provide a developmental framework for discussing reported patterns at a point in time, not a diagnosis, fixed label, or permanent classification.

Your participation in ILCI-21 assessment is entirely voluntary. You are not required to complete the assessment, and you may withdraw at any time before submitting your responses without penalty or negative consequence. Withdrawing from the assessment will not affect your employment status, standing within your organization, or access to other platform features.

If you choose not to participate or wish to withdraw after submission, please contact your organization's account administrator or curtis@isozakicoaching.com.

When you complete ILCI-21, we collect:

• Your responses to the 21 assessment items (ratings on a 1–7 agreement scale)
• The date and time your assessment was completed
• Your user identifier and organizational role, used to associate your responses with the correct team and organization in order to generate team and organizational reports

We do not collect:

• Personality diagnoses, clinical psychological data, psychiatric evaluations, or medical information
• Biometric or physiological data of any kind
• Data from any third-party assessment (MBTI, CliftonStrengths, Enneagram, etc.), ILCI-21 is a standalone instrument
• Any information beyond what you actively enter into the Platform

Your assessment responses are used for the following purposes:

• Individual results: Your responses are scored to generate a personal leadership reflection profile, accessible to you in your individual results dashboard and downloadable as a PDF report.
• Team reports: Your responses are aggregated with those of other team members to produce team-level culture insights. Individual responses are not identified in team reports. Reports are suppressed when fewer than three respondents have completed the assessment to protect anonymity.
• Organizational reports: Responses are further aggregated at the organization level to inform executive and administrator dashboards and downloadable PDF reports.
• Coaching recommendations: Your subdomain scores are used to surface personalized Isozaki Coaching service recommendations (e.g., workshop suggestions) on your dashboard. These are developmental suggestions only, not prescriptions or diagnoses.
• Service improvement: De-identified, aggregated data may be used by Isozaki Coaching LLC to improve assessment validity, scoring methodology, and platform features.
• Research and case studies: De-identified, aggregated results may be used by Curtis Isozaki and approved colleagues at Isozaki Coaching LLC for research, scholarly publication, conference presentations, white papers, and case studies. No individually identifiable information, employer name, or other identifying details will be disclosed in any published research or case study without separate, explicit written consent.

Assessment results are not intended and must not be used as the sole basis for employment decisions, promotions, demotions, terminations, performance evaluations, or legal proceedings. They are organizational development tools designed to facilitate constructive team and leadership conversations.

We take steps designed to protect the confidentiality of your assessment responses. Individual-level data is accessible only to you and, where your organization has configured access, to designated administrators. We do not share individual assessment data with third-party vendors, employers, insurers, government agencies, or other parties except as required by law or as explicitly consented to by you. For full details, see our Privacy Policy.

You have the right to:

• View your individual assessment results at any time through your personal dashboard
• Request a copy of your data in portable format
• Request correction of factual inaccuracies in your profile information
• Download a machine-readable copy of your data through the authenticated Platform
• Submit or cancel an account deletion request, which is reviewed and scheduled after a 30-day grace period
• Withdraw assessment consent; withdrawal is recorded as a new, timestamped consent-history entry

Deletion removes identifiable assessment data unless an active legal hold or other legal obligation requires a temporary pause. It does not affect prior lawful processing or irreversibly de-identified aggregate statistics. You may use the authenticated Platform, contact your organization's account administrator, or contact curtis@isozakicoaching.com.

ILCI-21 assessment provides a structured framework for understanding leadership and culture perceptions at a point in time. Scores reflect self-reported perceptions and are subject to individual interpretation biases, response tendencies, and contextual factors. Isozaki Coaching LLC makes no guarantees regarding organizational outcomes, culture change, individual performance improvement, or any other result from assessment participation or use of assessment reports.

Isozaki Coaching LLC is not a licensed clinical psychologist, psychiatrist, or mental health provider. Nothing in ILCI-21 platform or its reports constitutes clinical advice, diagnosis, or treatment.

By accepting these terms within ILCI-21 before your first assessment, you acknowledge that:

• You have read and understood this Consent document
• Your participation is voluntary and you may withdraw before submission; after submission, you may record withdrawal and request deletion as described above
• Your responses will be used as described above for organizational development purposes
• Results will not be used as the sole basis for employment decisions without your explicit additional consent
• ILCI-21 is an original instrument and is not affiliated with any third-party assessment framework
• You agree to the Terms of Service and Privacy Policy
